Community Resource Specialist

Description

HealthFirst is seeking a Community Resource Specialist to join our team of mission-minded professionals. While Community Health Worker certification is not required at the time of hire, the successful candidate must obtain certification within one year. This is an excellent opportunity for someone who can hit the ground running and provide hands-on support to patients needing help with insurance applications, sliding-fee programs, and navigating community resources.

Key Responsibilities

• Assist patients with insurance and benefits navigation (Marketplace, NH EASY, Medicaid, etc.)
• Process sliding-fee applications, discuss payment options, and arrange payment plans
• Ensure timely follow-up on applications, referrals, and patient contacts
• Provide patient education and advocacy related to community resources (food, housing, transportation, childcare, etc.)
• Support community outreach events, including occasional evenings/weekends
• Collaborate with billing and patient services teams to resolve coverage or payment issues
• Verify and update demographics, consents, and related documentation during each interaction
• Serve as a knowledgeable point of contact for patients needing assistance throughout Open Enrollment

Requirements

Required

• High school diploma or equivalent
• Community Health Worker (CHW) certification — or ability to obtain certification within one year of hire
• Minimum of 2 years’ experience in a healthcare, human services, or insurance benefits setting
• Strong customer service skills with clear, compassionate communication
• Solid computer proficiency and comfort learning new systems
  

Preferred

• Certified Application Counselor (CAC) designation through CMS, or willingness to obtain
• Familiarity with medical terminology
• Experience working with diverse populations and strong cross-cultural communication skills
