Baystate Health is seeking a Registered Nurse to join our Hospice Team. Our Hospice Team provides compassionate, patient-centered end-of-life care to patients and their families in the home and community setting, working closely with physicians, advanced practice providers, interdisciplinary care teams, patients, and families to deliver expert symptom management, emotional support, and high-quality care focused on comfort, dignity, and quality of life.

The Registered Nurse role offers the opportunity to provide meaningful, holistic care to patients and families during the end-of-life journey while developing specialized skills in hospice nursing, coordinating care with an interdisciplinary team, and making a lasting difference in the lives of patients and those who support them.

The Registered Nurse Will:

• Provide compassionate, patient-centered nursing care to individuals with advanced illness, focusing on comfort, dignity, and quality of life.
• Assess patients’ physical, emotional, and psychosocial needs and develop individualized plans of care in collaboration with the interdisciplinary team.
• Monitor and manage symptoms such as pain, dyspnea, nausea, anxiety, and other end-of-life concerns using evidence-based nursing interventions.
• Administer medications and treatments safely while educating patients, families, and caregivers on medication use, symptom management, and changes in condition.
• Communicate regularly with physicians, nurse practitioners, social workers, chaplains, aides, and other members of the hospice care team to coordinate services.
• Provide education, guidance, and emotional support to patients and families throughout the hospice experience, including preparation for changes associated with end-of-life care.
• Respond to changes in patient status and provide timely clinical interventions, including coordination of urgent and after-hours hospice needs when appropriate.
• Accurately document assessments, interventions, patient responses, and care coordination in accordance with clinical, regulatory, and hospice standards.

• Schedule: 8a-4:30p (start and end at home)
• Weekend Requirement: Every 6th Weekend

Compensation

Note: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range. Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.
