Concord Post Acute is seeking a compassionate, dependable, and skilled Licensed Vocational Nurse (LVN) to join our team on a full-time basis. Key Responsibilities

• Provide compassionate nursing care to residents in accordance with their individualized care plans.

• Administer medications and treatments as ordered.

• Monitor residents' health status and report changes promptly.

• Perform nursing assessments and document care accurately.

• Communicate effectively with physicians, residents, families, and members of the interdisciplinary team.

• Assist with care planning and implementation of individualized treatment plans.

• Supervise and provide direction to CNAs as appropriate.

• Maintain accurate and timely clinical documentation.

• Follow all facility policies, procedures, and applicable state and federal regulations.

• Promote resident safety, dignity, comfort, and quality of life.

Qualifications

• Current and valid California LVN license.

• Current CPR certification.

• Previous skilled nursing or post-acute experience preferred.

• Strong clinical, communication, and organizational skills.

• Ability to work effectively in a fast-paced team environment.

• Compassionate and professional approach to resident care.
