Heartline Staffing is recruiting a Primary Care Physician for a full-time opportunity in Jacksonville, Florida, focused on comprehensive care for senior patients. This physician will manage acute and chronic conditions, build trusting relationships with patients, and lead a collaborative care team with accountability for patient outcomes.

POSITION OVERVIEW
This role combines comprehensive primary care with health coaching, care coordination, team leadership, and a strong focus on patient outcomes. The physician will assess, diagnose, treat, manage, educate, and support patients while participating in clinical, quality, and risk-management initiatives designed to promote better health.

COMPENSATION
• $221,141-$315,915 Annual Salary

ROLE
• Provide Comprehensive Care For Patients With Acute And Chronic Illnesses
• Maintain Accountability For Patient Outcomes
• Assess, Diagnose, Treat And Manage Patient Conditions
• Provide Patient Education, Health Promotion And Care Coordination

Requirements

REQUIREMENTS
• MD Or DO In Internal Medicine, Family Medicine, Geriatrics, Or A Similar Specialty REQUIRED
• Current Active Florida Medical License REQUIRED
• Proficiency In Evidence-Based Medicine REQUIRED
• Commitment To Continuous Learning And Improvement REQUIRED
• Strong Leadership Skills And Ability To Positively Influence Patients And Teams REQUIRED
• Proficiency With Microsoft Office Suite, Including Word, Excel, PowerPoint And Outlook REQUIRED
• Fluency In Written And Verbal English REQUIRED

PREFERRED
• Board Certification In Internal Medicine, Family Medicine, Geriatrics, Or A Similar Specialty
