About the Role

Responsible for developing and implementing strategies that promote coordinated care across various health and wellness services for better customer experience and health outcomes which leads to a more sustainable group medical portfolio.

Ability to navigate complex healthcare environments, foster collaboration among diverse stakeholders, and drive initiatives that improve the quality and efficiency of care services.

Customer Centricity - This role contributes to the delivery of customer outcomes that are reliable, thoughtful, and create meaningful impact for customers and society. The role holder is expected to embed a customer first mindset in all decisions and actions by developing a clear understanding of customer needs, taking end to end ownership to resolve issues, and working collaboratively across teams to continuously enhance the customer journey—both directly and indirectly.

Responsibilities:

Program Development and Implementation

  • Design, implement, and evaluate innovative coordinated care programs that align with organizational goals and improve health outcomes for our members.
  • Utilize advanced data analytics to track program performance, identify trends, and develop actionable insights that inform strategic decisions.
  • Tailor programs to address the unique needs of various populations, ensuring equity in care delivery.
  • Implement mechanisms for continuous monitoring and evaluation of care coordination efforts.
  • Lead quality improvement projects focused on enhancing care processes, increasing patient engagement, and reducing readmission rates.
  • Promote adherence to clinical guidelines and best practices among care teams.
  • Ensure that all coordinated care initiatives comply with Hong Kong regulations, as well as internal policies and procedures.
  • Identify potential risks and develop mitigation strategies to protect the organization and its members.
  • Build and maintain strong partnerships with internal departments (HIS, operations, claims, and customer service) and external stakeholders (healthcare providers, community organizations, and agencies) to ensure a cohesive care delivery model.
  • Facilitate regular meetings and workshops to promote collaboration and share best practices.

Training, Education, and Capacity Building

  • Develop and deliver training programs for staff and providers on care coordination principles, patient engagement strategies, and effective communication techniques.
  • Cultivate a culture of continuous learning and improvement within the care coordination team.

Requirements:

  • Bachelor’s degree in nursing, Healthcare Administration, Public Health, or a related field; Master’s degree preferred.
  • Minimum of 10 years’ experience in healthcare management, with at least 3 years in health insurance sector.
  • In-depth knowledge of healthcare delivery systems, care coordination models, and quality improvement methodologies.
  • Demonstrated analytical and problem-solving skills, good organizational, interpersonal, and team skills.
  • Demonstrate expertise with Microsoft.
  • Flexible and able to work in a dynamic environment and across cultures.
  • Excellent verbal and written communication skills in English
  • Strong and effective interpersonal relationship skills.
