Description

• Oversee and direct the daily work of Certified Nursing Assistants (CNAs) and Medication Aides, ensuring assigned care and documentation are completed accurately and in compliance with established standards.
• Assess residents for changes in physical, mental, or emotional condition; document findings and promptly report significant changes to the Registered Nurse (RN), physician, or Nurse Practitioner.
• Process, transcribe, and carry out physician and nurse practitioner orders, ensuring timely communication and follow-up.
• Administer prescribed medications and treatments accurately and safely, observe residents for therapeutic and adverse responses.
• Provide resident and family education regarding care plans, treatments, and changes in condition; maintain effective communication with families and interdisciplinary staff.
• Participate in the development, implementation, and ongoing review of individualized resident care plans and specialty assessments such as skin integrity, hydration, pain, and safety.
• Monitor residents during meal times to ensure adequate intake, safety, and adherence to prescribed dietary needs; report concerns to dietary, RN, or physician.
• Assist residents with activities of daily living as needed and promote independence, dignity, and quality of life.
• Investigate and document incidents, accidents, or unusual occurrences; collaborate with the RN and Social Services for follow-up and resolution.
• Participate in nursing rounds, shift reports, and interdisciplinary team meetings to ensure continuity of care and communication.
• Maintain infection control and safety standards, including use of universal precautions and proper handling of biohazard materials.
• Monitor and document the use and maintenance of medical equipment and supplies, such as oxygen, IVs, and specialty devices.
• Attend required in-service education programs and contribute to the orientation and training of new nursing staff.
• Maintain accurate and timely nursing documentation in electronic and paper health records according to facility policy and nursing home regulations.
• Respond to medical emergencies and perform resuscitation or other emergency procedures within the scope of practice.
• Perform related duties as assigned to support quality resident care and facility operations.
High school diploma or equivalent. Graduate of an accredited school of practical nursing required. One (1) year of clinical nursing experience in a long-term care, skilled nursing, or similar healthcare setting preferred. A combination of education and experience may be considered. Current licensure as a Licensed Practical Nurse (LPN) in the State of Wisconsin required and maintained in good standing. CPR and First Aid certification required.
• Duties require frequent standing, walking, bending, stooping, kneeling, pushing, pulling, and reaching throughout the workday.
• Must be able to lift, move, and reposition residents or equipment weighing up to 50 pounds independently and more with assistance or mechanical lift devices.
• Regularly assists in resident transfers using proper body mechanics and assistive equipment to prevent injury.
• Requires visual and auditory acuity sufficient to monitor resident safety, recognize changes in condition, and accurately read and document information.
• Work involves potential exposure to infectious diseases, bloodborne pathogens, cleaning chemicals, and other occupational health hazards; appropriate personal protective equipment (PPE) must be used as required.
• May be exposed to varying temperatures and noise levels in resident rooms, dining areas, and common spaces.
• Requires manual dexterity for operating medical and mobility equipment, documenting in electronic health record systems, and performing resident care tasks.
• Work schedule may include days, evenings, nights, weekends, and holidays based on facility needs.
• Risk of bloodborne pathogen exposure is considered high.
