Job Summary
We are seeking a compassionate and relationship-focused Clinical Liaison to coordinate seamless transitions of care from hospitals and skilled nursing facilities to hospice services. This role combines clinical assessment, discharge planning, patient education, admissions support, and referral relationship development to ensure exceptional patient outcomes and continuity of care.
Key Responsibilities
Develop and maintain relationships with hospitals, SNFs, physicians, and other referral sources.
Conduct onsite patient assessments following hospice referrals.
Review physician orders, evaluate clinical needs, and collect referral documentation.
Educate patients and families regarding hospice services.
Coordinate safe transitions from acute or post-acute settings to home-based care.
Collaborate with physicians, case managers, social workers, and facility staff on discharge plans.
Ensure physician oversight and required orders for home health and hospice plans of care.
Communicate referral and clinical information to interdisciplinary care teams.
Support hospice admissions.
Help reduce avoidable hospital readmissions and improve patient outcomes.
Qualifications
Current and unrestricted California Registered Nurse (RN) of Licensed Vocational Nurse (LVN) License.
Experience in home health, hospice, case management, discharge planning, care coordination, or clinical liaison services preferred.
Strong communication, relationship-building, and patient education skills.
Ability to work independently while managing multiple priorities.
Proficient in clinical documentation and interdisciplinary collaboration.
