Key Responsibilities
Evaluate patients and develop individualized treatment plans.
Provide skilled physical therapy services in accordance with physician orders and care plans.
Supervise Physical Therapist Assistants (PTAs) in compliance with state practice regulations.
Collaborate with interdisciplinary team members to coordinate and optimize patient care.
Participate in care conferences, rehabilitation meetings, and utilization reviews as needed.
Maintain timely and accurate documentation, including evaluations, treatment notes, progress reports, and discharge summaries.
Educate patients, families, and caregivers on treatment goals, mobility strategies, safety techniques, and home exercise programs.
Assist with discharge planning and recommendations for continued care.
Recommend and coordinate appropriate durable medical equipment (DME).
Ensure a safe treatment environment by monitoring and reporting equipment maintenance needs.
Qualifications
Current Physical Therapist (PT) license in good standing.
Strong communication, organization, and interpersonal skills.
Passion for delivering exceptional patient-centered care.
Ability to work effectively within a multidisciplinary team.
