Description
Physical Therapist (PT) – Home Health | Poughkeepsie, NY | Full-Time, Part-Time & PRN.
Stern at Home Therapy is hiring Full-Time, Part-Time, and PRN Physical Therapists (PTs) to provide individualized, one-on-one care to patients throughout Poughkeepsie, NY, and surrounding communities.
Whether you're looking to build a full-time caseload, maintain consistent part-time work, or supplement your existing schedule with flexible PRN visits, Stern offers multiple options based on your availability and career goals.
Our model lets therapists build a geographically concentrated caseload, minimizing unnecessary travel while providing individualized care in patients' homes.
What You'll Do:
- As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population while maintaining clinical autonomy over evaluations and individualized plans of care.
Responsibilities include:
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments in patients' homes.
- Help patients improve strength, balance, mobility, endurance, and functional independence.
- Provide gait training, transfer training, therapeutic exercise, and fall-prevention interventions.
- Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
- Educate patients and caregivers on exercises, mobility strategies, and safety.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, families, and interdisciplinary care team members.
- Coordinate with our administrative team regarding scheduling and patient care.
Compensation:
Full-Time – Minimum 16 Patients.
Full-Time W-2 Salary:
- $92,000–$110,000 annually.
- Salary may increase based on patient volume.
Full-Time W-2 Tiered:
- $55–$70 per follow-up visit.
- $75 per 4-unit evaluation.
- Your per-visit rate increases as your patient volume grows.
Full-Time 1099 Tiered:
- $70–$80 per follow-up visit.
- $85 per evaluation.
- Your per-visit rate increases as your patient volume grows.
- Annual earning potential up to $145,000.
Part-Time – Minimum 10 Patients.
Part-Time W-2:
- $65 per follow-up visit.
- $75 per 4-unit evaluation.
- Fixed per-visit compensation with no tiered volume model.
Part-Time 1099:
- $72 per follow-up visit.
- $85 per evaluation.
- Fixed per-visit compensation with no tiered volume model.
PRN – Minimum 5 Patients.
PRN 1099 Only:
- $72 per follow-up visit.
- $85 per evaluation.
- Fixed per-visit compensation with no tiered volume model.
Full-Time W-2 Benefits:
- Health insurance and/or medical stipend.
- 401(k) with annual employer match starting at 1.5% and increasing up to 7%.
- 2 weeks of PTO after 6 months, increasing to 3 weeks after 9 months.
- 5 paid holidays, effective immediately, with flexibility to choose which days you use.
- Student loan repayment assistance beginning after the first year at $125 per month.
- $500 annual CEU stipend.
- $250 monthly commuter stipend for full-time visits.
- Equipment provided.
- Childcare assistance program currently in development.
Part-Time W-2 Benefits:
- Health insurance and/or medical stipend based on eligibility.

- Prorated commuter stipend based on patient volume, up to $175 per month for clinicians seeing 10 patients.

- 1099 independent contractors are not eligible for W-2 employee benefits.

Qualifications:
- Active Physical Therapist license in New York required.
- Graduate of an accredited Physical Therapy program.
- Reliable transportation required.
- Strong clinical, communication, documentation, and time-management skills.
- Ability to work independently in a home-based environment.
- Home health or geriatric experience preferred but not required.
