Senior Transitional Liaison, Registered Nurse

As Senior Transitional Liaison, Registered Nurse, you’ll contribute to our success in the following ways:
- Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
- Assesses, interprets, and manages patient/client needs with the intent of providing appropriate service.
- Monitors current referred patients to determine if the appropriate level of care is being provided.
- Maintains daily communication with the department team leader
- Maintains visibility and accountability to increase relationships within the company.
- Acts as a liaison with other transitional liaisons, direct-care clinical teams, clinical supervisors, physicians, and team members on behalf of providers and patients/families.
- Provides consultation on appropriate care services and/or specialty programs.
- Participates on committees and special projects.
- Assesses eligibility and provides updates and recommendations for care planning of new patients to the appropriate clinical team.
- Obtains data on physical, psychological, social, and other factors that may influence patient/family health status and shares with the clinical team.
- Participates in orientation and ongoing education programs.
- Performs other duties/projects as assigned.
What is Required?
- Graduate of an accredited Certificate, Diploma, Associates or Bachelor’s Degree in Nursing
- 3+ years experience with home health, hospice, geriatrics or general medical.
- Experience in surgical nursing or critical care is preferred.
- Current state license as an RN.
- Demonstrates excellent verbal and written communication skills
- Understands specialty programs and services available for patients.
- Proficiency with MS Office Suite (Word, Excel, Outlook, etc.)
You will report to the Transitional Care Manager.
