The Community Health Coordinator role in Greenville, NC supports grant-funded community health initiatives by maintaining internal tracking systems, managing and analyzing program data in Excel, and ensuring accurate reporting and spend-down across multiple projects. The position collaborates closely with the Community Impact Director and grants administration to support administrative functions, including purchasing program supplies (e.g., blood pressure cuffs) and refining data management processes for efficiency and accuracy. 

 

It also plays a key external-facing role by engaging community partners, distributing forms, and following up to ensure timely and complete data submission. In addition, this position helps coordinate and build a developing network of volunteers and ambassadors to strengthen community outreach efforts. This is a part-time hybrid position expected to work 25 hours per week, with some travel required to connect with community partners. 

 

This position is grant-funded and currently scheduled to end on June 30, 2029.

Responsibilities

1. Internal Tracking, Data Management & Grants Support

  • Maintain and update internal tracking systems, including Excel-based spreadsheets and other program databases, ensuring accuracy and consistency across grant-funded projects.
  • Support grant administration processes by tracking spend-down, monitoring budgets, and coordinating with internal stakeholders to ensure compliance and timely reporting.
  • Assist with procurement and ordering of program supplies (e.g., blood pressure cuffs), ensuring timely fulfillment and proper documentation.
  • Identify opportunities to refine and improve existing tracking mechanisms, increasing efficiency and reliability of data management processes.

2. Data Collection, Reporting & Process Improvement

  • Collect, organize, and validate data from internal sources and community partners, maintaining data integrity across multiple workflows.
  • Prepare reports, summaries, and basic analyses to support program tracking, outcomes measurement, and grant deliverables.
  • Develop and maintain standardized tools (e.g., forms, templates, and tracking documents) to streamline data submission and reporting.
  • Support ongoing process improvement efforts related to data collection, tracking, and partner reporting systems.

3. Community Partner Coordination & External Engagement

  • Serve as a point of contact for community partners, supporting coordination efforts related to program activities and data submissions.
  • Distribute forms, surveys, and reporting requests to partners and follow up to ensure timely and complete responses.
  • Maintain strong working relationships with external stakeholders and provide clear communication to support program participation.
  • Coordinate logistics and communication with community partners to support successful project implementation.

4. Volunteer & Ambassador Program Support

  • Assist in the development and coordination of a volunteer and ambassador network to support community outreach initiatives.
  • Support recruitment, onboarding, and ongoing communication with volunteers and ambassadors.
  • Help organize activities, assignments, and engagement efforts to ensure volunteers are effectively utilized and supported.
  • Track participation and contributions to evaluate and strengthen the program over time.

5. Other Duties as Assigned

  • Provide additional support to Community Impact team projects as needed.
  • Assist with administrative tasks, special initiatives, and cross-functional activities that advance grant-funded and community health goals.

 

Qualifications

  • High school diploma or equivalent and 3 years minimum of prior related experience. 
  • Progressive experience in public health, community health or equivalent
  • Proven ability to recruit, train, mobilize and manage volunteers
  • Demonstrated ability to drive toward and achieve results
  • Effectively manage relationships with the ability to gain trust and demonstrate influence without authority
  • Knowledge of community networks
  • Completion of a Community Health Worker training or willingness to complete one. 
  • Proficiency in Microsoft Office applications
  • Ability to reliably travel to meeting/training sites and availability to coordinate schedules with participating organizations and volunteers, including outside of regular business hours (weekends/evenings). This position will require the Coordinator to attend occasional Saturday events.

 

Here are some of the preferred experiences and skills we are seeking:

 

  • Knowledge of the social determinants of health (SDOH) as it relates to behavior modification through self-care activities and policy/system-wide changes.
  • Understanding of local health systems, relationships with hospitals, health clinics, and Federally Qualified Health Centers preferred

Compensation & Benefits

The pay rate is fixed at $28.00/hourly per the terms of the grant funding the position.  American Heart Association reserves the right to pay more or less than the posted range.
