Role Overview 

The Therapy Care Coordinator (TCC) is a team member who serves as the primary liaison for physical and occupational therapy orders. In this role, you will help patients navigate their care options, drive clinic capture rates, and deliver exceptional service from the moment a physician prescribes therapy through the duration of their treatment. 

Key Responsibilities 

• Patient Engagement: Welcome patients with therapy orders, tour facilities, introduce therapy staff, and coordinate scheduling for inbound and outbound appointments within 48 hours. 
• Care Coordination: Oversee the therapy script queue, verify insurance benefits/authorizations, manage reminder calls, and send clinical documentation (POCs, re-evaluations, progress notes) to referring physicians. 
• Analytics & Support: Track daily, weekly, and monthly clinic metrics, maintain internal tracking systems, and assist with department operations and FOTO administration. 
• Cross-Departmental Collaboration: Work closely with physical therapists, clinical staff, patient access, and billing teams to ensure smooth transitions and continuous care quality. 

Requirements

•  High School Diploma or equivalent. 
• Strong organizational, analytical, and prioritizing skills with the ability to make independent decisions. 
• Excellent interpersonal and communication skills across diverse teams and patients. 
• Proficiency with personal computers and standard office software applications. 
• Passion for customer service, confidentiality, and professional growth.
