As we prepare to open the Ng Siu Chan Centre, our newest long-term care home, we are seeking passionate and dedicated IPAC Manager who want to help shape the culture, resident experience, and care environment from day one.

Reporting to the Executive Director the IPAC Manager will ensure that there is an organized infection prevention and control program that meets regulatory requirements and is in line with IPAC standards. The IPAC Manager will implement and oversees infection prevention and control best practices that promote the safety of residents, staff volunteers, visitors and physicians across all Yee Hong sites.
Qualifications:
Current Registered Nurse, Registered Practical Nurse, Allied Health Professional or equivalent, in good standing.
Current CPR certificate
Infection Prevention and Control (IPAC) certificate
Certification in Infection Control (CIC), through Infection Prevention and Control Canada (IPAC) required or a willingness to obtain within next 1 year.
Nursing experience in a supervisory, educator position, preferably in Long-Term Care (LTC) or Complex Continuing Care, with relevant experience working in infection control and prevention is required.
Previous experience in long-term care or health care sector is highly preferred
Solid understanding of adult learning principles and teaching methods to facilitate learning preferred
Ability to organize, plan, and work independently
Ability to manage multiple priorities in a fast paced environment
Strong computer skills, including but not limited to Point Click Care and MS Office suite
Ability to think critically, analyze information and problem solve effectively
Flexible, team-player, able to build and maintain positive working relationships
Good knowledge of current directives and legislation in long term care
Ability to work with minimal supervision and demonstrate sound decision making, time management and administrative skills.
Strong interpersonal skills, ability to exercise empathy and discretion, patience and oral and written communication skills to effectively communicate with residents, the public, staff and external service providers.
Satisfactory Police Reference Check (with vulnerable sector screen) result
Fluency in Cantonese/Mandarin is an asset
Responsibilities:
1. Core Functions
Supports and assists the Executive Director as the Corporate IPAC chair in the development and review of IPAC policies, checklists, quality improvement initiatives, data collection and analysis.
Acts as a member of the broader site management team and provide on-call support during evenings and/or weekends;
Support the Executive Director and Director of Resident Care (DRC) in conducting employee investigations, recruitment and performance management as required;
Lead the Divisional IPAC Quarterly meeting including facilitation of the meeting and development of reports and policy updates. .
Collaborates with the Director of Quality and Performance on enterprise wide research IPAC initiatives
Ensures the evaluation and action plan for improvement of the Infection Prevention and Control Program is conducted annually
Participate and Lead various divisional and corporate committees.
Collaborates with Facility Manager to ensure cleaning protocols are developed based on best practices.
2. Outbreak Prevention & Management
Collaborates with Public Health, Hospital IPAC Hub and Divisional Leadership team to manage outbreaks
Coordinates and Chairs Outbreak Management meetings with local Public Health and other required stakeholders during an outbreak, including minutes, tracking of follow-up actions and implementation and oversight of Outbreak Control Measures
Develops and updates an accurate daily Outbreak Line List
Ensures timely communication with Public Health re infectious diseases as per regulatory requirements
Monitor and supports audit of key IPAC practices; present data to staff, management and to the Joint Health and Safety Committee; facilitates corrective action as needed, including direct coaching and management of staff related to compliance gaps;
Works with the Director of Resident Care, Executive Director and the pandemic communications lead to ensure timely communication to residents, staff, volunteer, physicians, visitors and families related to potential and confirmed outbreaks
Provides education to Residents, volunteers, learners and visitors related to infection control principles;
Conducts a post outbreak analysis and report for the IPAC Committees.
Conducts the orientation/mentoring of new IPAC champions within the organization
3. Surveillance & Monitoring
Daily supervision of IPAC activities in the home
Assists the Infection Prevention and Control champions in the homes to coordinate and implement infection control practices
Ensures surveillance and screening program in place for residents and team members
Reviews surveillance data and monitors for infection trends
Collaborate with the inter-professional team and all divisional departments when implementing IPAC best practices for Yee Hong Consults with the physicians/Nurse Practitioners and pharmacist on laboratory results and trending of data to promote antibiotic stewardship
Conducts routine audits of the home with increased frequency during outbreaks to ensure that IPAC processes, compliance to PPE, donning and doffing and hand hygiene practices are being adhered to by all staff, physicians and visitors
Prepares an action plan for gaps identified through surveillance / audit activities
Assists the ED, DRC, ADRC and IPAC champions in collaboration with Hospital IPAC Hub of the Ontario Health Team (OHT) and Public Heath to manage outbreaks, ensure timely communication with Public Health, line list is updated daily and accuracy of data is communicated to internal and external partners.
Ensures regular outbreak meetings occur during an outbreak, documented minutes and follow up actions
Supports management with post outbreak analysis and trending.
Conducts Respiratory Fit Testing training and ensures compliance to training requirements.
4. Mentoring
Provides education and coaching to team members on IPAC practices
In collaboration with others, conduct in-depth investigations of clusters/outbreaks of infections and collate appropriate data.
Participates in the planning and development of a pandemic plan and surveillance program for residents, staff, and visitors;
Assists the leadership team with IPAC education, orientation and training of employees
Ensures IPAC, environmental practices, and outbreak protocols are implemented and followed by all staff.
Educates staff on the principles and practices of infection prevention and act as a resource regarding infection prevention and control
Ensures that in-service programs are provided and encourage the continuing development of the nursing and environmental staff.
Provides education to Resident and Family Councils; liaises with the Public Health Unit, and the LTC homes partner hospital or OHT
Leads and/or conducts IPAC audits, PPE donning/doffing audits, and environmental audits to ensure compliance with the IPAC protocols and environmental cleaning practices according to PIDAC best practices guidelines, Corporate IPAC policies, and the LTC homes Act.
Chairs the Corporate IPAC committee and other committees/Task Force as required
Leads the planning, implementation, maintenance and administration of the Influenza Vaccination Program;
In collaboration with Occupational health and wellness, coordinates and facilitates mask fit testing as required
Collaborates with Public Health regarding communicable diseases and partner hospitals regarding transfers of cases.
Conducts or participates in interviews, orientations, and evaluations as needed
Liaise with all managers and interprofessional team to ensure holistic, safe, and quality care
Assists with ordering and maintenance of IPAC inventory and supplies
Assists in the development and updating of evidence-based Nursing Policies and Procedures in accordance with best practice guidelines and participate in relevant nursing research.
Develops Environmental Services policies and procedures in accordance with best practice guidelines, public health recommendations and relevant infection control protocols.
Other duties as may be assigned from time to time.
