Description
At CareWorks, we are seeking a compassionate, dedicated, and competent Home Health Licensed Nurse to join our dynamic team. Our ideal candidate must deliver the best healthcare services to our patients in their homes, facilitating a healthy, safe environment, enhancing their quality of life. Responsibilities: Conduct patient evaluations, focusing on their physical, emotional, and social needs with an aim to devise personalized care plans. Provide guidance and educate patients and their families about health conditions and the appropriate ways to manage them. Carry out regular visits to patients' homes to monitor their progress and reaction to treatments and caregiver techniques. Perform routine procedures including administering medications, dressing wounds, and providing acute care as needed. Maintain accurate, detailed records of patients' history, symptoms, and care provided, ensuring confidentiality and compliance with regulations. Coordinate and communicate effectively with the healthcare team, including doctors, therapists, and case managers, advising on patient conditions and needs. Qualifications: Active state License Practice Nurse (LPN) or Registered Nurse (RN), with a clean record. Minimum of 2 years of nursing experience required, ideally in a home health setting. Outstanding interpersonal, communication, and case management skills. Working knowledge of health care legislation, professional standards, and best practices in nursing. Proficient in medical software and MS Office and able to adapt to new technologies. Available for regular visits to patients' homes, having a driver's license and reliable transportation.
