Job Description

This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.
This position is a PRN role that will provide weekday and weekend coverage as needed to our Inpatient Child and Adolescence Unit at the Institute of Psychiatry. Weekday availability required. License at the time of hire required.
Additional Job Description
Education: Master's degree in a social work or social welfare program accredited by the Council on Social Work Education. Credentials: MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.
