Job Description

The Associate Healthcare Director supports the Healthcare Director in leading the community’s health and wellness services and ensuring that resident care operations are safe, efficient, person-centered, and compliant. This position helps with resident assessments and service plans, medication management, team member training, documentation, infection prevention, and survey readiness. The Associate Healthcare Director works closely with community leadership, residents, families, physicians, pharmacies, and other healthcare partners to promote resident independence, safety, and quality of life.

Essential Functions:

 

Resident Care & Service Planning

• Evaluate prospective residents before move-in to determine whether their care and service needs can be met by the community, and complete reevaluations as required by changes in condition or applicable regulations.

• Develop individualized service plans based on resident assessments, needs, and stated preferences, and revise plans as conditions or regulatory requirements change.

• Collaborate with residents, families, physicians, pharmacists, third-party healthcare providers, and other members of the healthcare team to coordinate care and services.

• Communicate compassionately and promptly with residents and families, respond to questions or concerns, and participate in effective problem-solving.

Medication Management, Compliance & Quality

• Participate in oversight of the community’s medication management program and promote safe, accurate medication administration.

• Administer medications and perform medication-related duties within the scope of the required Medication Aide license and applicable state regulations.

• Coordinate with the pharmacy, provide medication-management training and skills validation, and conduct routine medication and documentation audits.

• Maintain working knowledge of applicable local, state, and federal requirements and Harmony Senior Services policies and standards.

• Ensure resident assessments, service plans, medication records, and other required documentation are complete, accurate, timely, and survey-ready.

• Participate in developing and carrying out plans of correction, when applicable.

• Maintain infection prevention and control standards to prevent or limit the spread of communicable illness.

Team Leadership & Community Support

• Provide training, coaching, and skills validation for medication technicians, medication aides, personal care aides, nursing assistants, and other resident care team members.

• Support interviewing, onboarding, orientation, and ongoing development of resident care team members.

• Create and maintain the resident care team schedule, subject to the Healthcare Director’s oversight and approval, to support appropriate staffing coverage and community needs, including helping secure coverage for callouts, up to and including working the shift when necessary.

• Participate in community and resident care meetings and provide leadership coverage when the Healthcare Director is unavailable.

• Partner with the Healthcare Director and Harmony Square Director to support residents with behavioral expressions and other wellness needs.

• Promote a collaborative, respectful, and service-oriented culture across the community.

Requirements:

• Licensure: Must hold and maintain a current, valid, and unrestricted Medication Aide license in the state where the community is located. Continued employment in this position is contingent upon maintaining the required license in good standing.

• Current CPR certification is required.

• One to two years of experience in senior living, assisted living, memory care, medication administration, or a related resident care setting is required; three or more years of relevant experience is preferred.

• Experience supporting older adults and individuals living with Alzheimer’s disease or other forms of dementia is strongly preferred.

• Knowledge of medication administration practices, resident assessment and service planning, infection prevention, documentation standards, and applicable regulatory requirements.

• Strong organizational, time-management, verbal communication, and written communication skills.

• Ability to handle confidential resident and team member information with professionalism and discretion.

• Ability to work a flexible schedule, including days, evenings, nights, weekends, and holidays as business needs require.

• Ability to participate in an after-hours on-call rotation and serve as Manager-on-Duty on selected weekends.

Physical Requirements:

• Must be able to sit, stand, walk, bend, kneel, use standard office and resident-care equipment, and communicate effectively in person, by telephone, and electronically.

• Must be able to lift up to 50 pounds.

• Must be able to respond to resident care needs throughout the community and work in an environment that may require frequent movement and occasional extended periods of standing or walking.

• Must possess and maintain a valid driver’s license and meet applicable company driving requirements when driving is an assigned job function.
