Sign-On Bonus

Registered Nurse (RN)

• Full-time: $5,000
• Part-time: $2,500

Licensed Practical Nurse (LPN)

• Full-time: $3,500
• Part-time: $1,750

Starting pay is based on licensure, experience, qualifications, internal equity, and local market conditions.

Chestnut Family Health Center is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) in Bloomington, Illinois.

This is an opportunity to continue doing meaningful, hands-on nursing while enjoying a schedule that leaves room for life outside of work. You'll have a consistent weekday schedule with no nights, weekends, or holidays, along with generous paid time off and opportunities for tuition reimbursement and loan repayment.

Responsibilities

• Carry out individualized healthcare plans developed by practitioners.
• Administer medications and treatments prescribed by practitioners.
• Interview patients and gather relevant health histories.
• Administer and evaluate medical tests as appropriate.
• Provide health education to patients and their families.
• Assess patients by phone and in person for acute physical health concerns.
• Communicate patient needs and concerns to appropriate members of the healthcare team.
• Serve as a liaison among prescribers, patients, laboratories, primary care providers, other healthcare providers, pharmacies, and Chestnut staff to support continuity of care.
• Provide support and guidance to patients while helping maintain a safe and therapeutic environment.
• Document patient care and related information accurately in the electronic medical record.
• Protect the confidentiality and security of patient and organizational information.
• Promote Chestnut's culture of customer service excellence through adherence to behavioral standards.
• Perform other duties as assigned.

Qualifications

Licensure & Education

• High school diploma or equivalent required.
• Active Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required.
• CPR certification required or ability to obtain certification within 90 days of hire.

Skills & Knowledge

• Knowledge of pharmacology and potential medication side effects.
• Strong patient assessment and communication skills.
• Ability to provide patient and family education in a clear, supportive manner.
• Ability to collaborate effectively with providers, pharmacies, laboratories, and other members of the healthcare team.
• Basic computer skills, including Microsoft Word and Outlook.
• Experience with electronic medical records (EMR) and e-prescribing systems.
• Strong organizational skills and attention to detail.
• Ability to provide compassionate, professional care to individuals from diverse backgrounds.
