Position Summary
We are seeking a compassionate and skilled Physical Therapist (PT) to join our rehabilitation team on a PRN basis. In this role, you will evaluate and treat residents, develop individualized therapy plans, and collaborate with physicians, families, and interdisciplinary team members to maximize resident mobility, function, and independence.
Key Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide skilled physical therapy services in accordance with physician orders and established goals
- Assess functional abilities, mobility, strength, balance, and endurance
- Monitor resident progress and modify treatment plans as appropriate
- Communicate effectively with residents, families, physicians, nursing staff, and interdisciplinary team members
- Complete accurate and timely documentation in compliance with facility, state, federal, and payer requirements
- Participate in care planning meetings and discharge planning activities
- Educate residents, families, and caregivers on treatment plans, safety techniques, and home exercise programs
- Conduct in-service education and training for facility staff as needed
- Recommend adaptive equipment and interventions to improve safety and independence
- Maintain compliance with all facility policies, state practice acts, and regulatory requirements
Supervisory Responsibilities
- Supervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state regulations and facility policies
Qualifications
Education & Experience
- Bachelor's degree in Physical Therapy required
- Master's or Doctorate degree in Physical Therapy preferred
- Experience in skilled nursing, rehabilitation, or long-term care preferred
- Proficiency with Net Health and PointClickCare (PCC) preferred
- Strong clinical, communication, and documentation skills
Licenses & Certifications
- Current Physical Therapist license in South Carolina and in good standing
- Must maintain all required continuing education requirements
- Must remain in good standing with the applicable State Board
