Licensed Practical Nurse Home Health

Job Description:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team. In this role, you will provide direct patient care in the home setting under the direction of a Registered Nurse and in collaboration with physicians and other healthcare professionals. You will support patients and caregivers through skilled nursing services, patient education, medication management, and ongoing monitoring, helping patients achieve the highest level of independence and comfort in their own homes.

Shift Details: Monday through Friday day shift

Location: Las Vegas Area

Minimum Qualifications:

• Current LPN License in the state of practice.
• Basic Life Support for Healthcare Providers (BLS-HCP) certification upon hire OR obtained within 60 days of hire with prior approval. Advanced Care Life Support (ACLS) is required if applicable to unit.
• Approved IV Certification, as required by facility.
• Clean driving record, as required by facility.
• Ability to connect to the company’s online database twice daily, as required by facility.

Preferred Qualifications:

• One (1) year of nursing experience preferred.
• Previous home health, hospice, long-term care, or community-based nursing experience preferred.
• Experience providing patient and caregiver education preferred.
  

Patient Care Responsibilities:

• Providing skilled nursing care in patients' homes.
• Monitoring patient conditions and reporting changes to the RN case manager or physician.
• Assisting with medication management and patient education.
• Reinforcing treatment plans and teaching patients and caregivers.
• Performing wound care and other treatments within LPN scope of practice.
• Documenting visits electronically and coordinating care with the interdisciplinary team.
• Traveling between patient homes throughout the service area.

Physical Requirements

• Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
• Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
• Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
• Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
• Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
• May be expected to stand in a stationary position for an extended period of time.
• For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$26.74 - $41.28
