  POSITION SUMMARY:


The Refugee & Migrant Long-Term Case Manager will provide holistic and Long-Term case management to eligible refugee & Migrant clients that have been identified as especially vulnerable. Case Manager will complete self-sufficiency plans at intake as well as periodic assessments throughout the client service term to achieve long-term self-sufficiency. Successfully promote and support the mission of Catholic Charities of Northeast Kansas as guided by the Catholic Social teachings through the services outlined in the Refugee & Migrant Services. ESSENTIAL FUNCTIONS:

  • Provide case management services for a caseload of refugees and immigrants participating in the Long-Term Case Management (LTS) program.
  • Ensure participants are well-versed in their LTS Program rights, responsibilities and ensure services are well-documented track a participants' progress in electronic case files.
  • Adhere to Catholic Charities of Northeast Kansas’s Refugee & Migrant Services guidelines on service provision and documentation. Learns and implements guidelines of Refugee & Migrant services for the Long-Term Case Management set forth by Refugee & Migrant Service Program. 
  • Applies strengths-based approach in case management in efforts support especially vulnerable clients to obtain self-sufficiency. 
  •  Receives referrals for case management for -eligible clients and participates in any needed case conference meetings to assure continuity of care.
  • Assesses client needs, works with the client to create Self-Sufficiency Plan (SSP), and regularly meets with all clients on the assigned case load in alignment with CCNEK’s Refugee & Migrant Program policy for the Long-Term Case Management.
  • Collaborates with internal and external partners to ensure client is receiving holistic support to reach goals and achieve long-term self-sufficiency and integration.  
  •  Respond to crises with appropriate resources and in a timely manner to stabilize client – maintaining safety and ethical boundaries. 
  •   Support clients in vulnerable situations – including, but not limited to, domestic violence, substance abuse, homelessness, and mental health concerns, 
  •   Provide advocacy for clients as well as support accessibility to services, including transportation, language access, and community partner education as necessary. 
  •   Conduct home and field visit assignments as needed and in accordance with the agency health and safety protocol.
  •   Maintains professional conduct with clients as identified in the NASW Code of Ethics and abides by all program policies and procedures and acts as a mandated reporter.
  •   Must complete VIRTUS training, background screening, drug testing, and any ongoing training as required.
  •   Perform any other functions as may be directed by the employer.


Requirements

  

MINIMUM POSITION REQUIREMENTS:

  •  Bachelor’s Degree in social work or human service field or comparable experience is preferred
  •  Must have competency in oral, written, and interpersonal communication skills in English; fluency in a second language (Burmese, Karen, Dari, Swahili, etc.) preferred
  •  Ability to work effectively in a multi-cultural and multi-lingual environment
  •  Strong typing and internet skills as well as the ability to learn Microsoft Outlook, Excel and various database programs
  •  Must be able to work collaboratively with outside agencies and advocate on behalf of the clients
  •  Must be reliable, punctual, professional, and respond appropriately to people in crisis
  •  Must have a valid driver’s license, car insurance, a verified safe driving record and reliable means of transportation for local travel; must provide a copy of license and insurance
  •  Demonstrate a passion for the mission of Catholic Charities
  •  Must be willing to perform by the agency’s values: love, patience, kindness, faithfulness, self- control and joy

PHYSICAL DEMANDS:

  • Prolonged periods of sitting at a desk  and working on a computer, standing, bending and moving
  • Must be able to lift up to 10 pounds 
  • Must have the physical ability to make visits to a variety of environments that may include walking on rough terrain and climbing stairs
  • Must have physical ability to effectively communicate with others
  • Must have the physical ability and appropriate licensure for driving
