Overview

The Clinical Staff Pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states. Clinical Staff Pharmacist proficiently provides direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel. 

Responsibilities

Core Responsibilities

• Responsible for properly assessing patients' pharmaceutical needs and providing pharmaceutical care.

• Responsible for the accurate and proper compounding and dispensing of prescribed medications and solutions.

• Responsible for building and maintaining partnerships with patients and other healthcare providers to help build a strong multidisciplinary healthcare team that works to improve patient outcomes.

• Responsible for properly counseling patients, patients' families, and appropriate healthcare professionals regarding drug therapy.

• Responsible for daily pharmacy workflow and pharmacy operations in the absence of the Pharmacist-in-Charge or designee.

Pharmacy Operations

• Supervises and performs functions involved in the accurate and efficient dispensing of prescriptions.

• Reviews, fills, and verifies medication orders for accuracy and completeness prior to dispensing.

• Monitors patient drug therapy to detect and prevent drug interactions, contraindications, and other adverse reactions.

• Screens all new medication orders for appropriate dosing, allergies, drug interactions, and contraindications.

• Counsels patients, patients' families, and healthcare professionals on medication therapies and new medication orders.

• Makes appropriate clinical interventions to improve patient care and support guideline-directed therapy.

• Uses and maintains proficiency in the operation of pharmacy computer systems.

• Compounds and dispenses medications and pharmaceutical products.

• Obtains pertinent patient medication and medical information.

• Provides current and accurate drug information to patients and healthcare professionals.

• Communicates effectively with patients, healthcare professionals, and coworkers.

• Completes and properly documents medication therapy management interventions.

• Ensures compliance with company policies, legal requirements, procedures, and regulations.

• Ensures confidentiality of information, documentation, and records.

Specialty Pharmacy

• Ensures the pharmacy remains compliant with URAC accreditation standards and all applicable local, state, and federal regulations.

Retail Pharmacy

• Responsible for administering vaccines and other injections.

• Maintains and directs pharmacy operational workflow in the absence of the Retail Pharmacy Manager.

• Dispenses and/or administers medications to patients in accordance with ARH policies, including immunizations, naloxone, and injectable medications where permitted by law.

Hospital Pharmacy

• Supervises and performs functions involved in the IV Admixture Program, including the accurate and efficient preparation and delivery of total parenteral nutrition solutions, chemotherapy, antibiotics, and other medications for intravenous and irrigation therapy.

• Serves on hospital committees as assigned.

Additional Responsibilities

• Performs other related duties as assigned.

Qualifications

Education

• Bachelor of Science degree in Pharmacy or PharmD required.

Licenses & Certifications

• Current state pharmacist licensure in good standing required, or licensure eligibility in the state of employment within ninety (90) days of hire.

• Immunization certification required for Retail or Specialty Pharmacy positions.

Experience

• One (1) to two (2) years of pharmacy experience preferred.

Required Skills, Knowledge & Abilities

• Maintain the training and education needed to provide patient-centric pharmacy services.

• Retail Pharmacy: Obtain and maintain the training and education needed to provide comprehensive pharmacy services, including but not limited to naloxone administration, immunization certification, medication therapy management, and compliance with state and federal laws and regulations.

• Knowledge of pharmaceutical care principles, medication management, and patient counseling.

• Ability to assess patients' pharmaceutical needs and provide appropriate pharmaceutical care.

• Ability to accurately review, verify, compound, and dispense medications.

• Strong communication and interpersonal skills with patients, caregivers, providers, and healthcare team members.

• Ability to collaborate effectively within a multidisciplinary healthcare environment.

• Knowledge of state and federal pharmacy regulations and standards.

• Strong clinical judgment, critical thinking, and problem-solving skills.

• Ability to maintain accurate documentation and protect confidential patient information.
