Palliative care or hospice experience.
Make a meaningful impact on the lives of patients managing kidney disease. As a remote IKC Kidney Palliative Care Social Worker, your primary focus will be guiding patients and their families through the challenging stages of chronic kidney disease (CKD) and End Stage Kidney Disease (ESKD), including end of life. You will play a crucial part in their journey by driving Goals of Care (GOC) discussions, facilitating Advance Care Planning (ACP), providing anticipatory grief support, and addressing behavioral health concerns to ensure they receive the highest quality of care and emotional support.

Key Responsibilities:
Palliative Care Social Worker: Work directly with patients and families:
Palliative Care: Provide support to patients with advanced kidney disease and their caregivers through goals of care conversations, palliative social work assessment, behavioral health interventions, and advance care planning.
Education and Advocacy: Educate patients, families, and caregivers about available community resources and palliative and hospice care services. Advocate for patient needs and preferences.
Team Collaboration: Collaborate with interdisciplinary teams to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients. Liaise with dialysis clinic staff, nephrology practices, and outside providers to accomplish goal-concordant care for patients.
Qualifications:
Education: Master’s degree in Social Work from an accredited institution.
Licensure: State licensure as a Licensed Clinical Social Worker (LCSW) or equivalent is required. Active license in the state of California.
Experience:
One year of hospice or palliative care experience required.
At least 6-8 years of social work experience, preferably in healthcare settings.
Two years of renal experience preferred.
Skills: Strong interpersonal and communication skills; ability to provide quality patient care in a remote work setting; thorough assessment skills.
Knowledge: Familiarity with palliative care principles, grief counseling techniques, advance care planning processes, and behavioral health interventions.
Attributes: Empathetic, self-directed, adaptable, and able to handle emotional situations with professionalism.

The Salary Range for the role is $78,000.00 - $94,000.00 per year.
