Job Summary

The Ambulator Clinical Pharmacist will collaborate with the primary care team at Summit Pacific Medical Center (SPMC) to provide medication and chronic disease state management with an emphasis on optimizing medication therapy and safety for patients.

Job Duties and Responsibilities

Ambulatory pharmacist patient visits
Medication consults with primary care providers
Participation in the development of chronic disease treatment protocols

Providing medication information and education to primary care team

Provide medication dosing support for SPMC Anticoagulation Clinic

Cross-training with SPMC Pharmacy Department

Special projects and other related duties as assigned.

Organizational Responsibilities

In addition to the duties and responsibilities listed above, all employees are expected to support the Summit Pacific Medical Center mission, vision and values; comply with SPMC policies and procedures; and conduct themselves in an ethical, professional, respectful, and collaborative manner at all times.

Required Education and Experience

  • PharmD from an accredited school of pharmacy
  • A post-graduate year residency or two years’ experience in the ambulatory clinical pharmacy field

Required Licenses, Certifications and/or Registrations

Active Washington State Department of Health pharmacist’s license

Preferred Board-Certified Ambulatory Care Pharmacist (or equivalent)

Current BLS certification.

Required Knowledge, Skills, Abilities

Ability to work collaboratively in a multidisciplinary team of health care providers

Skilled in-patient communication and education

Skilled in-patient assessment, medication adjustment, monitoring, and all other relevant aspects of patient care.
