Primary Care Nurse Practitioner – Queens, New York

Position Summary
The facility is seeking a Primary Care Nurse Practitioner to join its growing primary care team in Queens, New York. This is a full-time opportunity focused on delivering comprehensive, patient-centered primary care in an outpatient clinical setting.
The Nurse Practitioner will evaluate, diagnose, and manage patients across a broad range of acute and chronic medical conditions while working collaboratively with physicians, advanced practice providers, nurses, and experienced clinical support staff.
Essential Position Responsibilities
- Provide comprehensive primary care evaluations, including patient histories and physical examinations.
- Diagnose and manage acute and chronic medical conditions across a diverse adult patient population.
- Develop and implement individualized treatment plans based on patient needs.
- Provide preventive healthcare services, health screenings, and routine wellness examinations.
- Manage chronic conditions including diabetes, hypertension, hyperlipidemia, cardiovascular disease, and other common medical conditions.
- Order and interpret laboratory tests, diagnostic studies, and other appropriate evaluations.
- Prescribe medications and provide ongoing medication management.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Provide patient education regarding disease prevention, healthy lifestyle choices, medication adherence, and chronic disease management.
- Coordinate care with physicians, specialists, behavioral health providers, and other healthcare professionals.
- Provide referrals and follow-up care when appropriate.
- Maintain accurate and timely documentation within the electronic health record (EHR).
- Participate in care coordination, quality improvement, and population health initiatives.
- Promote continuity of care and establish strong, long-term relationships with patients.
- Provide culturally sensitive, compassionate, and patient-centered care to the Queens community.
Qualifications
Education
- Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
- Graduate-level preparation in Family Practice, Adult-Gerontology Primary Care, or a related primary care specialty.
Certification
- Board Certification as a Nurse Practitioner through ANCC, AANP, or another nationally recognized certifying organization.
- Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) certification preferred.
Licensure
- Active or eligible Nurse Practitioner license in New York.
- Active Registered Nurse (RN) license in New York.
- DEA registration or eligibility to obtain.
Experience
- Experience in outpatient primary care, family medicine, internal medicine, or community health preferred.
- Experience managing acute and chronic medical conditions preferred.
- Strong clinical assessment, communication, and patient education skills.
- Ability to work effectively within a multidisciplinary healthcare team.
- New graduates with strong clinical training in primary care may be considered.
