Project Manager & Data Specialist – Community Health Workers | Full-Time (80 hours per pay period)Requirements:
- Bachelor's degree in a relevant field required
- 3-5 years of relevant management experience required
- Previous experience in understanding and addressing disparities in maternal and infant health preferred
- Strong understanding of healthcare systems
- Previous experience with electronic medical record systems and data analysis tools preferred
- Awareness of health equity principles and ability to segment findings by race, gender, ethnicity, and other key areas of difference among Bronson patients.
- Ability to summarize qualitative and quantitative data into key findings and action-oriented recommendations.
- Ability to collaborate with a variety of Bronson departments and the Bronson Health Foundation.
- Demonstrated ability to plan and facilitate progress meetings with system leadership.
- Ability to develop and implement feasible work plans.
- Interpersonal skills to effectively interact with all levels of Bronson personnel and community leaders.
- Detail and process oriented
Responsibilities:
- Manage the day-to-day duties of Community Health Workers
- Understand barriers to care including access to primary care, prevention, and health literacy
- Analyze demographic data for patient intake, patient outcomes, and patient experience to identify disparities within delivery of care to patients. Leverage system resources to identify disparities that are rooted in inequity.
- Plan community outreach and patient advisory sessions to gain additional information.
- Utilize data from primary care in regard to social determinants of health and how they correlate to health outcomes.
- Work as a member of various Bronson Care teams with a connection to Bronson Engagement and Community Partnership team and support Community Health Worker program objectives as needed.
- Develop recommendations for programming, outreach, and training that address inequities in Bronson’s delivery of care and improve quality of care over time.
