Case Manager, Service Coordination Team
Annual Salary: $50,604
Work Schedule: Monday - Friday, 8:30 am - 5:00 pm
Hampton-Newport News Community Services Board (H-NNCSB) is seeking a Case Manager to join our Service Coordination Team. This team provides case management, crisis support, care coordination, and recovery-focused services to individuals transitioning from psychiatric facilities back into the community.
Case Managers provide ongoing monitoring and support to help individuals maintain stability, including supportive counseling, coping-skills education, recovery tools, crisis intervention, and connections to H-NNCSB and community resources. The team also provides valuable support to Reinvestment Case Management, Emergency Services, and other H-NNCSB programs, helping ensure continuity of care and coordinated services across the behavioral health system.
Required Qualifications
- Bachelor's degree in Human Services or a related field such as Social Work, Psychology, Sociology, or Counseling.
- One (1) year of experience in case management services.
What You'll Do
- Provide case management, care coordination, and supportive counseling to individuals transitioning from psychiatric facilities back into the community.
- Assess client needs and eligibility through interviews with clients, families, and care providers.
- Develop, implement, and review individualized treatment and service plans.
- Monitor client safety, progress, behaviors, medication compliance, and community stability.
- Provide coping-skills education, recovery tools, and support to promote ongoing stability.
- Coordinate referrals and connect clients with H-NNCSB and community resources, including housing, medical care, food, transportation, and benefits.
- Advocate for clients and serve as a liaison among clients, families, caregivers, healthcare providers, and community agencies.
- Schedule and coordinate appointments, home visits, and other services; visit client residences as needed.
- Participate in client review meetings, collaborate with other behavioral health professionals, and consult on complex cases.
- Maintain accurate and timely clinical documentation, reports, and case management records.
- Attend court hearings and provide testimony or other client-related information as required.
- Provide services to adults, depending on program needs.
