As a Home Health Occupational Therapist, you'll help patients regain the skills, strength, and confidence needed to perform everyday activities while remaining safe and independent in their own homes.

What You'll Do
- Complete occupational therapy evaluations, reassessments, and discharge visits
- Develop individualized treatment plans based on patient goals and functional needs
- Provide skilled occupational therapy services in patients' homes
- Assess home environments and recommend adaptive equipment and safety modifications
- Educate patients and caregivers on therapeutic techniques, home safety, energy conservation, adaptive equipment, and activities of daily living
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate with physicians, nurses, and the interdisciplinary care team
- Complete timely and accurate clinical documentation using WellSky
Qualifications
- Active Pennsylvania Occupational Therapist License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment, Communication, and Documentation Skills
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
