Posterity Group LLC is a veteran owned, service-disabled small business, headquartered in Rockville, MD, that specializes in federal government contracting and staffing. We are cultivating a WARP Multidisciplinary Clinic and looking to staff Case Manager to provide their services to veterans in our Rockville, MD headquarters. The Case Manager is expected to perform the following:
- Conduct comprehensive intake and social-needs screening.
- Develop and maintain individualized service-coordination plans.
- Schedule and track internal and external referrals.
- Coordinate insurance authorizations, benefits and appointment follow-up.
- Identify transportation, housing, food, caregiving, legal and financial barriers.
- Facilitate communication among providers, patients, families, schools and community agencies.
- Track emergency-department visits, hospital discharges and care transitions.
- Conduct outreach to patients at risk of disengagement.
- Prepare cases for interdisciplinary conferences.
- Maintain resource directories and closed-loop referral documentation.
- Support discharge and follow-up coordination.
- Escalate clinical concerns to licensed personnel.
- Avoid providing psychotherapy, diagnosis or licensed clinical services unless separately qualified.

Requirements
Minimum qualifications
- Bachelor’s degree in social work, psychology, rehabilitation, public health, nursing, human services or a related field.
- Two years of healthcare, disability, behavioral-health or community-services experience.
- Demonstrated knowledge of benefits, community resources and care coordination.
- Proficiency with electronic records and referral-management systems.
Preferred qualifications
- Master’s degree or relevant professional license.
- Certified Case Manager, Accredited Case Manager or comparable credential.
- Experience with VA Community Care, Medicare, Medicaid, commercial insurance or school systems.
- Bilingual or multilingual capability.
- Experience serving Veterans, children or individuals with disabilities.
