The Bronx Health Collective, an innovative, mission driven, federally qualified community health center affiliated with Montefiore Health System-Albert Einstein College of Medicine, is seeking a Clinical Social Worker to provide individual and group therapy, psychological assessments, and integrated mental health care to children, adolescents, adults, and families. This position will support the Bronx Health Collective (BHC) Terra Firma initiative and will focus primarily on providing psychotherapy for unaccompanied minors and families seeking asylum.
Terra Firma is a medical-mental health-legal partnership that provides wrap around services for unaccompanied minors and families seeking asylum, by providing psychotherapy, referrals to concrete resources, medical care, nutrition counseling, and care coordination, among other services. The Clinical Psychologist’s responsibilities would include providing individual and family psychotherapy, conducting evaluations, providing referrals to social services, and writing affidavits or letters of support as needed for immigration proceedings, in addition to seeing other clients within the larger BHC program.
Founded in 1987 to care for vulnerable families living in NYC’s homeless shelter system, BHC has grown to provide high quality, comprehensive primary care to over 9,000 children, adolescents, and adults annually. Since its inception, BHC has understood the importance of co-located, integrated mental health care, which is a fundamental part of its patient-centered model of care. Our mental health team consists of psychologists, clinical social workers, case managers, and patient navigators. BHC cares for NYC’s most vulnerable populations, with a cluster of health centers in the Longwood/Hunts Point section of the South Bronx and a homeless-health care program that provides care via mobile medical clinics or on-site in family shelters.
Clinical Social Worker Responsibilities:
Provides direct clinical care that may include individual, group, and family therapeutic interventions for children, adolescents, adults, and families.
Completes biopsychosocial assessments for new patients.
Write letters of support and/or affidavits for housing, immigration and other issues related to client mental health and well-being.
Documents care in a timely and appropriate manner using the electronic medical record (EMR).
Attends and actively participates in mental health team meetings and events.

Active New York State Licensed Psychologist.
Minimum of two (2) years of clinical experience preferred.
Excellent verbal and written communication skills.
Experience with social determinants of health preferred.
Demonstrated experience with underserved populations and community-based healthcare settings.
Demonstrated ability to collaborate across multidisciplinary care teams.
Bilingual (English/Spanish) required.
