Provision of social work services to adults, children and their families at The Ohio State University Total Health Care Center, a Federally Qualified Health Center with multiple sites. Core competencies include working with the integrated team to provide individual counseling, with a focus on substance use treatment and recovery support, as part of our mission to improve the mental and emotional well-being of the communities we serve.

Social work services include psychosocial assessments, mental health counseling, distress screening, crisis intervention, financial counseling, discharge planning, health education and the provision of material resources and linkage with community agencies.  Social work services are provided to patients/families to meet their medically related social and emotional needs as they impact on their medical condition, treatment, recovery, and safe transition from one care environment to another.

Participates in interdisciplinary patient care conferences and team meetings; serves as consultant to other professionals on the healthcare team; participates in staff meetings, in-services, continuing education programs, research activities, CQI and performance improvement activities; teaches and mentors health professional students, regardless of discipline, on theory and practice of care delivery by an inter-professional team; serves as preceptor for Community Health Worker and Social Work students. Possesses strong organizational and communication and is committed to community-based care.

 

Must meet mandatory education and health surveillance requirements.  Demonstrates competence in technical, interpersonal, and cognitive skills required to meet essential job functions. Demonstrates knowledge of the DSM-5 diagnostic criteria and co-occurring disorders.

Additional Information:

Minimum Education Required: Master’s degree in Social Work from an accredited institution. Current LISW license in the State of Ohio.

 

Minimum Years of Experience Required: Minimum of 1 year experience in individual therapy and substance use treatment.  2-4 years preferred.  Experience with Motivational Interviewing, CBT, DBY, or similar approaches preferred. Experience with visually impaired or people with disabilities.

Desired Qualifications: Current LPCC license in the State of Ohio.
