Human Touch Home Health is seeking compassionate and dependable Registered Nurses (RNs) for Full-Time and PRN opportunities with our Blacksburg team.
You'll provide skilled nursing care to patients throughout Blacksburg, Christiansburg, Radford, Pulaski, Floyd, and surrounding New River Valley communities.

What You'll Do
- Complete skilled nursing assessments and home visits
- Perform Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned
- Develop and update individualized plans of care
- Provide wound care, medication management, and disease education
- Monitor patient conditions and communicate significant changes
- Educate patients and caregivers
- Coordinate care with physicians, therapists, and other team members
- Complete accurate and timely documentation using WellSky
- Help patients safely manage their health at home
Qualifications
- Active Virginia Registered Nurse (RN) License Required
- Graduate of an accredited nursing program
- Current CPR/BLS Certification
- Valid Driver's License and reliable transportation
- Strong clinical assessment and communication skills
- Ability to work independently in patients' homes
- Home Health and OASIS experience preferred
- WellSky experience is a plus
