Help patients move from hospital care to healthier lives in the community.
ChristianaCare is seeking a Community Educator/Acute Care Connector to help patients navigate the often-complex transition from the hospital back into the community. In this highly collaborative role, you’ll work directly with patients and families to identify barriers that may affect their health—from transportation and access to follow-up care to food, housing and other social needs. You’ll connect patients with community resources, reinforce discharge plans and help ensure they have the support they need after leaving the hospital.

Work Hours: 8:00 am - 4:30pm
What You’ll Do
Screen patients for Social Drivers of Health (SDOH) needs and connect them with appropriate resources and services.
Help patients navigate health care, social services and community-based programs.
Support patients and families during the discharge process, reinforcing instructions using teach-back and helping address outstanding needs.
Ensure follow-up appointments are scheduled and help identify solutions to transportation and other access barriers.
Conduct follow-up outreach after discharge, including telephone follow-up and home visits for eligible patients.
Connect patients with internal and external resources, including referrals through Unite Us.
Collaborate with providers, caregivers and community partners to support safe, timely transitions of care.
Help patients identify social and economic challenges affecting their health and develop achievable health and social goals.
Build relationships with community organizations, faith leaders and other partners serving vulnerable and underserved populations.
Provide health education and participate in community outreach events.
Advocate for patients and help reduce cultural, socioeconomic and health-literacy barriers to care.
Document patient engagement and maintain required program data.
Qualifications:
High school diploma or equivalent. Bachelor’s degree preferred
At least one year of community-based and/or patient-facing experience, more preferred.
Public speaking, group facilitation and excellent writing skills preferred.
Computer experience required.
Demonstrated ability to work with diverse populations required.
Bi-lingual language skills are a plus.

Hourly Pay Range: $25.52 - $38.28This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting.
