DUTIES AND RESPONSIBILITIES:

The Mental Health Department provides recovery-oriented services, such as WRAP, Personal Medicine, Power Statement and Advanced Directives. All service delivery is based on the department’s guiding principles which include a focus on consumer strengths and choice.

The Apartment Counselor is responsible for providing direct care service to consumers in the CHIPP Supported Living Program.

Full-time Apartment Counselors will be scheduled to work a rotating shift consisting of three (3) working days on schedule and six (6) days off schedule.

When a shift falls on weekdays, i.e. Sunday through Thursday, the hours worked will be:

Work Time: 4:00 through 11:00 p.m. (7 hours)

Sleep Time: 11:00 p.m. through 7:00 a.m. (8 hours)

Work Time: 7:00 through 9:00 a.m. (2 hours)


When a shift falls on weekend day, i.e. Friday, Saturday, or on any Holiday, the hours worked will be:

Work Time: 4:00 through 11:00 p.m. (7 hours)

Sleep Time: 11:00 p.m. through 7:00 a.m. (8 hours)

Work Time: 7:00 a.m. through 4:00 p.m. (9 hours)



CRITICAL ELEMENTS:

1) Medication Monitoring: Implement the medication monitoring system as instructed (monitor medications taken, complete medication logs, etc.)

2) Direct Care: Respond to any consumer emergency during the shift and follow the program situation management directives. Provide supportive counseling when needed. Provide consumers with opportunities for educational and recreational activities. Provide consumers assistance with activities related to daily living skills (i.e. cooking, cleaning, laundry, socialization.)

3) Documentation: Accurately complete required documentation (i.e. daily progress notes in consumers’ logs, incident reports, communication logs, reporting PRN’s, etc.)

4) Consumer Safety: Make periodic checks on consumers, as needed, during the shift to ensure their safety



ADDITIONAL ELEMENTS:

1) Participate in Agency staff meetings, program staff meetings, trainings and special activities.

2) Contact supervisor for any emergency.

3) Transport CHIPP consumers as needed.

4) Serve actively as team member of the CHIPP Supported Living Program i.e. assist co-workers in providing coverage for sites during their absences; assist in providing training for new apartment counselors and assist with program projects.

5) Attend a minimum of 24 (twenty-four) hours of training per fiscal year.

6) Participate in quality improvement and consumer education efforts. Follow and support Agency philosophy, policies and procedures.

7) Assume other duties as assigned by the Supported Living Program Coordinator or Assistant Supported Living Program Coordinator.



Requirements

Minimum Qualifications: One (1) year College Credits in Social Science or one (1) year Human Service experience; 21 (twenty-one) years old; must have had license for 2 years, must have had license for 2 years, valid Pennsylvania Driver’s License and reliable transportation. Pennsylvania State Police Background Check. E.O.E.
