Social Services Director (Full-Time)
Diablo Valley Post Acute is seeking a compassionate, organized, and dedicated Social Services Director to join our team. This is an excellent opportunity for a professional who is passionate about advocating for patients and supporting residents and their families throughout their healthcare journey.
Position: Social Services Director
Employment Type: Full-Time
Salary: $80,000–$85,000 per year

Responsibilities
Assess residents' psychosocial needs and develop individualized care plans.
Coordinate discharge planning and transitions of care.
Provide emotional support, counseling, and advocacy for residents and families.
Collaborate with the interdisciplinary team to ensure quality resident care.
Connect residents and families with appropriate community resources and services.
Maintain accurate documentation and ensure compliance with state and federal regulations.
Participate in care conferences and quality improvement initiatives.
Qualifications
Experience in social services within a skilled nursing, post-acute, or healthcare setting preferred.
Knowledge of discharge planning, resident rights, and regulatory requirements.
Excellent communication, organizational, and interpersonal skills.
Ability to work collaboratively with residents, families, physicians, and healthcare professionals.
