Description
About the Role
MAC is looking for a compassionate, motivated Case Manager to connect people living with HIV to the care and support they need. You'll reach clients who are newly diagnosed, out of care, or hard to reach, then walk alongside them as they build the confidence and skills to manage their health. If you care about health equity and want work that makes a real difference in communities of color, we'd love to hear from you.
What You'll Do
- Do outreach in the community, at MAC's HIV screening services, clinics, and testing sites to engage new clients
- Help clients who are newly diagnosed, in care only sometimes, or lost to care get connected to services
- Offer emotional support as clients cope with a diagnosis and manage their care
- Help clients navigate medical and social services so they stay in care and build their own care coordination skills
- Advocate for clients inside the agency and with outside case managers and providers
- Support clients in developing harm and risk reduction strategies, and reinforce them over time
- Refer clients to in-house services such as Hep C, STI, and COVID testing and counseling
- Lead individual and group health education and risk reduction sessions
- Enroll clients and keep secure, confidential, up-to-date files in line with MAC protocols and HIPAA
- Take part in agency meetings, program planning, and professional development
What You Bring
- High school diploma or GED required, plus a BA in health, human services, or a related field, or 3 to 5 years of related experience
- Experience working with diverse and at-risk populations within communities of color
- Strong communication and time management skills
- Comfort working independently and as part of a team
- Group facilitation skills
- Strong computer skills
- Experience using social media for outreach is a plus
MAC is an equal opportunity employer. We strongly encourage people with lived experience and people from the communities we serve to apply.
