Location: Wayne County Jail -207 E Chestnut St Goldsboro, NC 27530
Pay: $18.00–$22.00 per hour

Noble Hearts HR Consulting & Staffing is seeking a compassionate, community-focused Care Manager Extender to support our Tailored Care Management team in helping members navigate complex healthcare, behavioral health, and social service systems.

The Care Manager Extender works under the direction and supervision of a Supervisory Care Manager and assists with member engagement, care coordination, resource navigation, and follow-up activities. This position helps expand care management capacity and ensures members receive the support and connections they need to access appropriate services and community resources.

The Care Manager Extender does not provide clinical services and must perform all assigned duties within the defined scope of Tailored Care Management functions.

Key Responsibilities

Member Engagement & Support

  • Assist with outreach and engagement activities to help members access needed services and supports.

  • Support members in understanding care plans, appointments, referrals, and available services.

  • Provide education and information regarding community resources and Medicaid services.

  • Help identify and address barriers to care, including transportation, scheduling, communication, and access to services.

  • Build positive, respectful relationships with members and support member-centered goals.

Care Coordination Support

  • Assist Care Managers with assigned care coordination and follow-up activities.

  • Document member interactions, contacts, and activities accurately in the care management system.

  • Communicate member needs, concerns, barriers, and updates to the assigned Care Manager.

  • Assist with follow-up related to referrals, appointments, service connections, and community resources.

  • Participate in team meetings and care coordination activities as requested.

Resource Navigation

  • Connect members with appropriate community-based services, programs, and support resources.

  • Assist members in navigating healthcare, behavioral health, social service, and community systems.

  • Use community knowledge and, when applicable, lived experience to help members identify and access resources.

  • Assist members with social determinants of health needs, including:

    • Housing

    • Food and nutrition

    • Employment

    • Transportation

    • Healthcare access

    • Other community and social supports

Compliance & Collaboration

  • Work under the direction and supervision of the Care Manager and perform only assigned and delegated functions.

  • Maintain member confidentiality and comply with HIPAA and organizational policies.

  • Ensure all activities remain within the approved Care Manager Extender scope of practice.

  • Complete and maintain all required training in accordance with applicable state and federal requirements.

  • Collaborate effectively with Care Managers, Supervisory Care Managers, and other members of the care team.

Minimum Qualifications

Candidates must meet all basic qualifications and at least one of the qualifying pathways below.

Basic Qualifications

  • Must be at least 18 years of age.

  • Must have a high school diploma or equivalent, such as a GED or certificate of completion.

Qualifying Pathways

Candidates must meet at least one of the following requirements:

1. Lived Experience with I/DD or TBI

  • Be an individual with lived experience with an intellectual/developmental disability (I/DD) or traumatic brain injury (TBI); and

  • Demonstrate knowledge of and direct personal experience navigating the NC Medicaid delivery system.

2. Behavioral Health & Peer Support

  • Be an individual with lived experience with a behavioral health condition; and

  • Be a Certified Peer Support Specialist.

3. Parent/Guardian Experience

  • Be a parent or guardian of an individual with an I/DD, TBI, or behavioral health condition; and

  • Have at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual.

Please note: A parent or guardian cannot serve as a Care Manager Extender for their own family member.

4. Relevant Paid Experience

  • Have at least two years of paid experience performing functions similar to those described under Care Manager Extender responsibilities; and

  • Have at least one year of paid experience working directly with the Tailored Care Management-eligible population.

Individuals who may be well-suited for this position include, but are not limited to:

  • Certified Peer Support Specialists

  • Community Health Workers (CHWs)

  • Individuals who have completed the NC Community Health Worker Standardized Core Competency Training (NC CHW SCCT)

  • Community Navigators who served in that capacity prior to the implementation of Tailored Plans

  • Family Navigators, as defined by an approved LME/MCO in accordance with applicable service descriptions

  • Professionals with relevant care coordination, case management, social service, or community resource navigation experience

Compensation & Benefits

Pay: $18.00–$22.00 per hour
