LPN Home Care Visits & Shifts – Stamford, CT
Looking for flexibility, competitive pay, and the opportunity to provide one-on-one patient care? Affirmed Home Care is expanding our team in Stamford and is seeking skilled Licensed Practical Nurses (LPNs) for per diem nursing visits and shift-based cases.

Pay Rates
- $125 per nursing visit
- $40/hour for hourly shifts

Qualifications
- Active Connecticut LPN License
- At least 1 year of recent clinical experience
- Home care experience is a plus
- Current BLS Certification
- Authorized to work in the United States
- Valid driver's license preferred
- Physical completed within the last 12 months
- Current PPD or QuantiFERON test, or chest x-ray if required
