Transition Coach – Home Stretch WA

Why this role matters 

Home Stretch WA is transforming what it means for young people to leave Out of Home Care. Instead of facing adulthood alone at eighteen, young people aged seventeen to twenty-one are supported, encouraged and championed as they build their futures. As a Transition Coach, you become one of the steady and trusted adults walking beside them during one of the most important transitions of their lives. 

You will meet young people where they are in their homes, in the community and at district offices. You will offer practical support, genuine connection and a belief in their potential. Every conversation, every visit and every moment of encouragement matters.

This is a full time, Monday to Friday outreach role based in Morley. Pay is in accordance with SCHADS Level 4. 

About the role 

As a Transition Coach, you will walk alongside young people as they move toward independence. You will provide one to one coaching, mentoring and case coordination that helps them build confidence, skills and strong support networks. You will support young people to stay connected to culture, country, family and community. You will help them access safe housing, education, training, employment and meaningful participation in their communities. 

You will work collaboratively with Child Protection staff, carers, kin, community members and other key stakeholders. Your practice will be trauma informed, culturally safe and strengths based. You will ensure young people’s voices shape their plans and goals. 

This role is primarily outreach based and may include home visits, in reach to hospitals, detention centres and regular co location at Child Protection District Offices. 

How you will make an impact 

  • Provide one to one coaching, mentoring and case coordination to young people transitioning from care 

  • Support young people to build natural networks and maintain connection to culture, country, family and community 

  • Assist young people to secure safe and stable housing and navigate the Home Stretch WA housing allowance 

  • Help young people access education, training, employment and meaningful community participation 

  • Walk alongside young people as they build independence, confidence and life skills 

  • Advocate strongly for young people and ensure their voices shape their plans and goals 

  • Work collaboratively with Child Protection staff, carers, kin, community members and other key stakeholders 

  • Maintain accurate case notes, records and outcomes data 

  • Participate in reflective practice, training and the Home Stretch WA Community of Practice 

What you will bring 

  • Relevant qualifications in youth work, community services or a related field 

  • Experience working with young people, ideally within the youth work or leaving care sector 

  • A deep understanding of the challenges faced by young people with care experience 

  • The ability to build trust, motivate and support young people through complex situations 

  • Strong case management skills and confidence working independently in an outreach capacity 

  • Excellent communication, relationship building and advocacy skills 

  • A trauma informed, culturally safe and strengths-based approach 

  • Flexibility, initiative and the ability to manage competing priorities 

  • A current WA Driver’s Licence 

  • A working with children and Departmental 395 checks are required 

  • A current first aid certificate is required
