About the Position
In collaboration with the multi-disciplinary Reid Hospice team, the Hospice Case Manager RN is responsible for coordination of all care for their assigned hospice patients; this includes admission, discharge, re-certification, and planning of care. The Case Manager RN will work closely with the Hospice Interdisciplinary Group (IDG), including patients and families, as well as Reid Health Leadership, Medical Staff, and physician’s practice office personnel; will work with representatives of the community, federal, and state agencies, as well as with outside review and accreditation agencies.
Overview of Responsibilities
Accepts patient assignments by Hospice Charge Nurse or Triage Nurse.
Uses assessment skills and interviewing techniques to identify problems and patient/family physical, psychosocial, spiritual, and educational needs.
Guides, supports, and provides education to hospice patients and families.
Develops and modifies the care plan according to the patient’s needs, expected outcomes to be achieved, and coordinates all ancillary services.
Carries out physician’s orders in compliance with policies and procedures.
Informs the physician(s) and other personnel of changes in the patient’s condition appropriately.
Manages patients’ pain and symptoms pursuant to physician’s orders; obtains changes in orders as needed, and updates the patient’s care plan and medication list accordingly.

Educa
Education Required: Must be a graduate of an accredited school of nursing.
Education Preferred: Bachelor of Science in Nursing.
Experience Required: Two or more years of clinical nursing experience.
Experience Preferred: Experience in Critical Care or Medical/Surgical; must have a broad understanding of age specific needs.
Licensure & Certification
Licensure: Currently licensed as a Registered Nurse in good standing in the State of Indiana.
Certifications: Must be Basic Life Support (BLS) Certified through American Heart Association.
