Job Summary:

The Admissions Coordinator is responsible for assessing, admitting, and transitioning patients into the appropriate level of care within the facility. This role involves initial patient evaluation, verification of medical history, and coordination with healthcare teams to ensure timely and accurate patient admissions. The Admissions Coordinator works collaboratively across multiple departments to provide seamless patient care and support optimal patient outcomes.

Key Responsibilities:

• Collaborates with physicians, care managers, and other healthcare staff to ensure accurate and timely patient admissions, including necessary documentation and care planning.

• Verifies patient information, including demographics, insurance, and medical records, to ensure compliance with facility admission protocols.

• Communicates effectively with patients and their families, providing information about the admission process, care expectations, and patient rights.

• Supports the coordination of care by initiating orders, treatments, and diagnostic tests as appropriate, based on the admission protocol.

• Maintains accurate and complete documentation of all admission activities in the electronic medical record (EMR), adhering to facility standards and regulations.

• Participates in quality improvement initiatives related to the admission process, identifying opportunities for enhancement and contributing to implementation efforts.

• Provides patient education on facility services, treatment plans, and post-admission care to ensure patient understanding and satisfaction.

• Ensures compliance with all regulatory requirements, including privacy, safety, and infection control, during the admission process.
