At Crossroads Hospice & Palliative Care, all our teams are integral parts of the compassionate care we provide to our patients and their families. While our caregivers offer invaluable medical support, the Palliative Care Coordinator's contribution is to ensure every aspect of our patients' needs is heard and met.  Our relationships are built on trust, serving as the linchpin of communication among our interdisciplinary teams.

Palliative Care Coordinator Qualifications:

  • Must possess a current, unencumbered LPN License
  • Experience in scheduling in a healthcare organization or unit
  • Receive and promptly process palliative care referrals and/or inquiries for services provided by Crossroads
  • Work in cooperation with the call center for incoming or received referrals.
  • Adept knowledge of the Palliative Care program and ability to explain to the fullest extent palliative care utilization and services available.
  • Maintain accurate and detailed records of all information calls and referrals.
  • Schedule and assign all palliative patients with designated Palliative Care Nurse Practitioners as appropriate.


Palliative Care Coordinator Schedule & Benefits:

  • Schedule: Monday to Friday, 8:30 AM - 5:00 PM
