- Consult with patients by phone or in person to determine specific needs, determine referrals to care.
- Build a patient-family centered relationship.
- Provide initial admission assessment, screenings, and assess for barriers to care.
- Report to team.
- Attend and/or review provider appointments; reinforce education and information provided by caregivers.
- Develop and deliver a Care Plan with treatment summary to patients and/or sign consents and declinations and reinforce education on treatments accepted or declined.
- Organize evidence-based care plan and present to patients for discussion.
- Follow up as required with patient and providers.
- Sustain a patient centered relationship with the patient and family throughout the course of care and into post-care as needed.
- 2 years of experience working directly with patients and providers required.
- 2 years of experience in specialty associated with application preferred.
- Minimum of Associate Nursing Degree for positions hired after 07/01/2018.
- Licensed as a Registered Nurse in the state of Colorado or from a valid compact state required
- Basic Life Support (BLS) required
- Department specific:
- If Oncology - Oncology Certified Nurse (OCN) preferred.
- If Obstetrics - Registered Nurse Certified – Inpatient Obstetric Nursing (RNC-OB) required

Pay is based upon relevant education and experience per hour.
