What You'll Do
- Provide skilled nursing care to hospice patients in their homes, assisted living facilities, or inpatient settings.
- Assess patient conditions, develop individualized care plans, and adjust treatments as needs change.
- Administer medications, manage pain and symptoms, and educate families on care techniques.
- Collaborate closely with physicians, social workers, chaplains, and other members of the interdisciplinary team.
- Offer emotional support and guidance to patients and their loved ones, helping them navigate end-of-life decisions.
- Maintain accurate, timely documentation in compliance with regulatory and organizational standards.
What You'll Bring
- Current RN license in the state of practice.
- Minimum of 1–2 years of nursing experience (hospice or palliative care preferred).
- Strong clinical assessment and critical thinking skills.
- Exceptional communication skills, with the ability to listen deeply and respond with empathy.
- Comfort working independently in varied home and facility environments.
- A valid driver’s license and reliable transportation.
