Reports To: Director/Manager of NHTD/TBI
Job Type: Full-Time
Salary: Up to $100,000 annually
Work Location: In Person
Travel: 75% within assigned territory
JOB SUMMARY:
Always Compassionate Health is seeking a Community Outreach Specialist – NHTD/TBI to develop and maintain referral relationships and increase awareness of our Nursing Home Transition and Diversion (NHTD) Medicaid Waiver and Traumatic Brain Injury (TBI) services throughout Westchester County and surrounding communities.
The Community Outreach Specialist will serve as a key community-facing representative of the NHTD/TBI program. This position will build relationships with hospitals, rehabilitation facilities, skilled nursing facilities, care management agencies, social workers, discharge planners, physicians, community organizations, and other referral sources to identify individuals who may benefit from NHTD/TBI services.
The ideal candidate will have a strong understanding of Medicaid-funded home and community-based services, NHTD/TBI waiver programs, referral and intake processes, and the needs of individuals with physical disabilities, traumatic brain injuries, and other qualifying conditions.
ESSENTIAL RESPONSIBILITIES:
Community Outreach & Referral Development
- Develop and maintain strong relationships with hospitals, rehabilitation centers, skilled nursing facilities, care management agencies, social workers, discharge planners, physicians, community organizations, and other referral sources.
- Conduct regular in-person outreach throughout the assigned territory to promote NHTD/TBI services.
- Identify and develop new referral sources and community partnerships.
- Educate referral partners and community organizations on NHTD/TBI eligibility, services, and referral procedures.
- Represent Always Compassionate Health at community events, healthcare meetings, networking events, resource fairs, conferences, and educational presentations.
- Develop outreach strategies based on referral trends, community needs, and program growth opportunities.
- Maintain consistent communication and follow-up with referral partners.
NHTD/TBI Program Education
- Educate prospective clients, families, caregivers, and referral sources regarding NHTD and TBI waiver services.
- Provide accurate information regarding program eligibility, Medicaid requirements, available services, and the referral/intake process.
- Assist individuals and referral sources in understanding the steps necessary to initiate the referral and enrollment process.
- Work collaboratively with Intake, Care Management, Clinical, and Program teams to facilitate appropriate referrals.
- Maintain current knowledge of NYS Medicaid, DOH, DSS, NHTD, and TBI program requirements and applicable regulations.
Lead & Referral Management
- Generate and track referrals and prospective clients within the assigned territory.
- Follow up with referral sources and prospective clients in a timely and professional manner.
- Maintain accurate records of outreach activities, referrals, appointments, and conversion outcomes.
- Monitor referral trends and identify opportunities to increase program enrollment.
- Communicate referral barriers, community needs, and growth opportunities to management.
- Provide regular reports regarding outreach activities, referral volume, conversion rates, and community engagement.
Community & Client Engagement
- Build rapport with individuals, families, caregivers, and community partners.
- Respond to questions regarding NHTD/TBI services and appropriately direct clinical or eligibility-specific questions to the appropriate department.
- Participate in educational presentations for community partners and prospective clients.
- Promote person-centered, community-based alternatives to institutional care.
- Maintain a professional presence that reflects the mission and values of Always Compassionate Health.
Territory Management
- Develop and maintain a structured outreach schedule for the assigned territory.
- Conduct frequent local travel to meet with referral sources, community partners, prospective clients, and other stakeholders.
- Identify underserved communities and potential referral opportunities.
- Monitor competitor and market activity within the assigned territory.
- Maintain an organized database/list of referral sources and outreach contacts.
QUALIFICATIONS:
- Bachelor's degree in Social Work, Human Services, Healthcare Administration, Business, Marketing, Public Health, or a related field required.
- Master's degree preferred.
- Minimum of 2 years of experience in community outreach, healthcare, social services, home and community-based services, care management, intake, or referral development.
- Experience with NHTD and/or TBI Medicaid Waiver programs strongly preferred.
- Knowledge of New York State Medicaid, DSS, DOH regulations, Medicaid eligibility, and community-based services.
- Experience working with individuals with disabilities, traumatic brain injuries, physical disabilities, or other complex healthcare needs preferred.
- Experience developing and maintaining relationships with hospitals, healthcare providers, social workers, care managers, rehabilitation facilities, and community organizations.
- Strong understanding of referral and enrollment processes.
- Excellent communication, presentation, networking, and relationship-building skills.
- Strong organizational and follow-up skills.
- Ability to work independently while collaborating effectively with internal teams.
- Proficiency with Microsoft Office and electronic documentation systems.
- Must have reliable transportation and a valid driver's license.
- Must be willing to travel approximately 75% within the assigned territory.
