Lead One of Queensland's Most Significant Healthcare Transformation Programs

About the Opportunity 

We are seeking an exceptional healthcare leader to join our growing Care at Home leadership team as QLD State Manager, Care at Home. 

Based in Brisbane, this is a Permanent Full-Time opportunity with responsibility for the operational leadership, growth and optimisation of Care at Home services across Queensland. 

Reporting to the National Director, Operations, you will lead a diverse portfolio of innovative in-home and virtual care programs, supporting both publicly and privately funded patients across a range of clinical streams. 

This role is responsible for driving operational excellence, workforce capability, clinical service delivery, stakeholder engagement and sustainable service growth across Queensland. 

Working closely with executive leaders, clinicians, hospitals, funders and strategic partners, you will play a pivotal role in designing and implementing new models of care while ensuring patients receive high-quality, safe and person-centred healthcare in their homes and communities.

Key Responsibilities 

Strategic Leadership & Service Growth 

• Lead the delivery, growth and optimisation of Care at Home services across Queensland, including Hospital in the Home (HITH), rehabilitation, aged care, palliative care, cancer care and cardiac programs. 

• Partner with Product, Growth and Clinical teams to design, implement and scale innovative home-based and virtual care services. 

• Develop and execute state-wide operational strategies that support sustainable growth, service excellence and organisational priorities. 

• Build strategic partnerships with hospitals, clinicians, insurers, Primary Health Networks, government agencies and other key stakeholders. 

Operational Excellence & Clinical Governance 

• Provide operational leadership across multidisciplinary teams, ensuring services are safe, effective, financially sustainable and aligned with clinical governance requirements. 

• Oversee workforce planning, service scheduling, resource allocation, performance management and operational delivery. 

• Monitor service performance, patient outcomes, risk, quality and financial metrics to drive continuous improvement. 

• Champion accreditation, compliance, quality and safety standards across all programs. 

Patient Experience & Innovation 

• Drive patient-centred models of care that improve access, experience and health outcomes. 

• Lead the integration of virtual care technologies, remote monitoring solutions and innovative service delivery models. 

• Promote evidence-based practice, integrated care pathways and continuous service improvement that reduces hospital admissions and supports care in the home. 

• Support evaluation, service redesign and healthcare innovation initiatives that enhance clinical and operational outcomes. 

People Leadership & Culture 

• Lead, develop and inspire high-performing multidisciplinary teams across Queensland. 

• Foster a culture of accountability, collaboration, innovation and continuous learning. 

• Build workforce capability through coaching, talent development, succession planning and performance management.

About You 

You are an experienced healthcare leader with a passion for healthcare transformation, operational excellence and patient-centred care. 

You bring strong commercial acumen, exceptional stakeholder management capabilities and a proven track record leading large-scale healthcare services through growth, change and innovation. 

You Will Bring 

• Significant senior leadership experience within healthcare services. 

• Demonstrated experience leading complex multidisciplinary healthcare teams and services. 

• Experience within community healthcare, hospital substitution, virtual care, aged care or home-based care models. 

• Proven expertise in operational management, service delivery and performance improvement. 

• Strong knowledge of clinical governance, risk management and accreditation requirements. 

• Experience developing and leading high-performing teams with a focus on engagement, culture and capability. 

• Demonstrated success building strategic partnerships with hospitals, clinicians, insurers, government and external stakeholders. 

• Experience driving innovation, service redesign and continuous improvement initiatives. 

• Strong understanding of virtual care platforms, patient navigation models and digitally enabled healthcare. 

• Sound financial, commercial and operational management skills. 

• Exceptional stakeholder engagement, communication and influencing capabilities. 

• Demonstrated ability to translate strategy into operational outcomes and measurable results. 

• High levels of resilience, professionalism, integrity and adaptability. 

Highly Regarded 

• Experience in Hospital in the Home (HITH), rehabilitation, aged care, oncology, palliative care, cardiac services or related healthcare sectors. 

• Experience establishing or scaling new healthcare services. 

• Understanding of public and private healthcare funding models. 

• Experience working within digitally enabled and innovative care environments. 

Qualifications 

• Tertiary qualifications in Nursing, Allied Health or another relevant clinical discipline. 

• Postgraduate qualifications in Health Management, Business Administration or a related field (highly regarded).
