Family/Adult Nurse Practitioner – Telehealth
About the Position
A leading healthcare organization is seeking an experienced Family or Adult Nurse Practitioner to provide virtual primary and urgent care to adult patients receiving home and community-based services.
This role focuses on supporting patients with chronic and complex medical conditions through individualized care plans, telehealth consultations, medication management, patient education, and coordination with physicians, specialists, families, and interdisciplinary care teams.
Key Responsibilities
- Provide primary and urgent healthcare through telephone and telehealth visits.
- Develop and manage individualized care plans for adults with complex and chronic conditions.
- Conduct patient assessments, review medical histories, and evaluate diagnostic information.
- Identify gaps in care and develop strategies to improve patient outcomes.
- Address patients’ physical, medical, and psychosocial needs.
- Educate patients and families regarding medications, treatment plans, nutrition, adherence, and disease progression.
- Collaborate with physicians, specialists, healthcare providers, and interdisciplinary teams.
- Maintain accurate clinical documentation and electronic medical records.
- Prescribe medications within applicable state regulations and prescribing authority.
- Participate in continuing education and professional development.
Requirements & Credentials
- Master’s degree in Nursing (MSN) required.
- Active national Nurse Practitioner certification.
- Current, unrestricted Family or Adult Nurse Practitioner license in New York.
- Current, active RN license.
- 2+ years of experience managing complex care cases.
- Experience working with Electronic Medical Records (EMR).
- Strong clinical assessment, communication, and care coordination skills.
Preferred:
- Medicaid registration.
- DEA registration or eligibility.
- Experience working with patients with developmental disabilities and/or chronic complex conditions.
