As a Patient Navigator, you'll help patients find their way through every step of their care journey. From coordinating appointments and connecting patients with community resources to following up after missed visits, you'll help remove barriers and ensure patients stay engaged in the care they need.

  • Help patients navigate healthcare services and connect them with the resources they need
  • Reach out to patients who miss appointments and encourage them to reconnect with care
  • Build relationships with community organizations and referral partners
  • Collaborate with providers and clinical teams to support patient care plans
  • Coordinate outreach events, educational programs, and community initiatives
  • Help organize patient support groups and educational opportunities
  • Maintain accurate patient records and documentation within the electronic health record
  • Coordinate meetings, communications, and special projects that support the program

  • Experience working with electronic health records (Epic experience is a plus)

šŸ“‹ Minimum Requirement

  • High School Diploma or GED (or equivalent experience)
  • OR 2 years of applicable work experience
