For more than 55 years, Family Health Centers of San Diego’s (FHCSD) mission has been to provide caring, affordable, high-quality health care and supportive services to everyone, with a special commitment to uninsured, low-income and medically underserved persons.

FHCSD is one of the top 10 largest federally qualified health centers (FQHCs) in the country. We operate more than 90 sites across San Diego County, including 29 primary care clinics, 23 behavioral health facilities, 10 physical rehabilitation clinics, nine dental clinics, five vision clinics, four outpatient substance use treatment programs, three mobile medical units, two mobile counseling centers, two urgent care centers, and a pharmacy.

Our staff provides care to over 227,000 patients each year, of whom 91% are low-income and 29% are uninsured. FHCSD provides care to all. Services include, but are not limited to adult care, chronic disease management, pediatrics, comprehensive women’s care including obstetrics, dental, vision, case management, physical rehabilitation, speech therapy for children, vaccinations, infectious diseases, behavioral health, substance use counseling and a host of specialty services including cardiology, podiatry, endocrinology, dermatology, among others.  FHCSD also offers supportive services to those who are unsheltered and in need of intensive case management.   

The breadth of our clinic locations, services and programs has grown over the last five decades, making us the largest community clinic provider of health care to the uninsured in the county and one of the top 10 largest community clinic organizations in the nation. We are also the largest health care safety-net provider, largest school-based health care provider and the largest mental health provider in the San Diego region.

Program Overview

Family Health Centers of San Diego’s Advanced Practice Provider (APP) Residency Program is a paid twelve-month educational program for recently graduated family nurse practitioners and physician assistants. The program offers a unique salaried opportunity to improve clinical skills and prepare for a career in community medicine by training within a federally qualified health center. The program will prepare APPs to manage complex care of patients in a multi-cultural, socioeconomically diverse population. Participants will also develop the broad skill base necessary to have a successful career providing primary care in outpatient settings to medically underserved communities.

Core training will consist of:

  • Orientation
  • Precepted clinical sessions
  • Didactics, journaling, case write-ups, and other clinical projects
  • Rotations/independent practice
  • Close mentorship from experienced clinicians
  • Ongoing evaluation to improve individual performance as well as enhance patient care

The next APP Residency Program cycle begins on November 2, 2026. Participants agree to a minimum 3-year commitment with FHCSD. Salary during the initial 12-month Residency Program is $125,000 annually. After graduation from the Residency Program, participants will transition to regular provider roles. Salary will be increased to $145,000 for the second year and $165,000 for the third of employment. Full-time employment is required.

Candidates must be willing to commute to any of FHCSD’s primary care locations.

Job Description

All FHCSD’s APP residents engage in daily clinical work and professional growth activities during the 12-month Residency Program. To be successful in this role you must be equally dedicated to both aspects of this position. Duties and Responsibilities

  • Maintain dedication to continuous learning
  • Provide quality and caring medical services to a diverse patient population
  • Perform routine exams, common procedures and screenings
  • Order and interpret laboratory tests, imaging, and point-of-care tests
  • Evaluate patients with complex problems and refer to appropriate specialists
  • Exercise cultural sensitivity and seek assistance with translations if needed
  • Complete all required documentation in accordance with departmental standards
  • Comply with electronic medical record requirements, including medication reconciliation, finalizing notes, clearing out personal pool, and participating in clinic pool
  • Attend all mandatory meetings and minimize incident reports
  • Use resources efficiently and practice medicine in a cost-effective manner
  • Participate in Clinical Outcomes Program
  • Maintain behavioral standards and respectful interactions with colleagues and patients
  • Obtain good peer reviews, attend mandatory meetings and minimize incident reports
  • Work toward meeting organizational targets for all metrics
  • Participate in administrative duties and on-call schedule in a timely manner
  • Must keep all required medical credentials active and in good standing
  • Perform other duties as assigned

Requirements

  • Current unrestricted license to practice in the State of California
  • Recent graduate with a master’s degree from an accredited nurse practitioner or physician assistant program
  • No experience required, less than 2yrs of licensed experience desired
  • Nurse practitioners must be graduates of a family nurse practitioner program
  • DEA eligibility
  • Willing to see patients of all ages and provide full-scope primary care
  • Commitment to a 2yr contract after successful completion of the initial 12-month APP Residency Program
  • Current American Heart Association health care provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training
  • Complete and pass national certification exams before application submission
  • Demonstrated commitment to community medicine and to serve homeless and medically underserved families.
  • Second language fluency desired, not required

Credentialing process will commence immediately upon acceptance.
