- Current state license as an Advance Practice Registered Nurse (APRN).
- Certification as a Nurse Practitioner.
- OR
- Current state license as a Physician Assistant
- Current certification from a recognized certifying organization such as the National Commission of Physician Assistants.
- AND
- Applicable experience as defined by position leadership
- AND
- Eligible for State Controlled Substance License II through V.
- Eligible for Federal DEA Certificate II through V.
- BLS Certification for Healthcare Providers.
- -2 years working experience as a provider in a Family Medicine clinic setting, will be the solo Family Medicine Provider
- -Schedule:-4 10's, M-F, one dedicated day off
