Nurse Practitioner / Advanced Practice Provider (APP)

Employment Structure & Schedule
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (Flexible). Includes shared telephonic on-call rotation to support 24/7 clinical stability for high-acuity residents.
About the Role:
Ohio State University Physicians (OSUP) is seeking a Nurse Practitioner to serve as a primary clinical lead within our Skilled Nursing and Sub-Acute Rehabilitation unit. This role is designed for a clinician who excels in a senior living setting, managing complex medical transitions for residents moving from acute hospital care to rehabilitation.
Key Responsibilities:
High-Acuity Management: Lead the medical bedside care for residents admitting to facility post-hospital for short term stay, including those with multiple comorbidities and post-surgical needs.Care Transitions: Conduct comprehensive admission assessments within 24–48 hours of arrival to ensure seamless transitions from hospital to skilled nursing facility to mitigate hospital rehospitalization along with ongoing management of care episode, and discharge planning.Interdisciplinary Leadership: Collaborate on patient care with physician, therapy, nursing, and social work to streamline discharge planning.Clinical Oversight: Order and interpret diagnostic tests, manage medication reconciliation, and perform minor bedside procedures (i.e. stitch removal) as needed. Perform comprehensive evaluations (H&P, progress notes, (re)certifications).Quality & Compliance: Partner with the Medical Director to ensure all documentation meets CMS Five-Star Quality Rating requirements and supports regulatory readiness. Document all patient encounters accurately and timely in the electronic medical record (EMR).Family Advocacy: Act as the primary clinical liaison for families, communicating effectively with patients and families to align medical interventions with goals of care.

Qualifications
Licensure: Active Ohio APRN license and national certification (AANP or ANCC).Experience: Minimum of 2 years of experience in hospitalist medicine, geriatrics, or sub-acute Rehab preferred.Competencies: Proficiency with PointClickCare or similar EMR; strong autonomous decision-making skills; cohesive collaborator and communicator; ability to thrive in a high(er)-volume, fast-paced environment.
